Keyword hierarchy
When I say keywords, I am not only talking about the text you put in the Meta tags on your page.
Keywords are all major terms and phrases, which describe the content of your web site.
Building a good keyword hierarchy is the most significant part of SEO.
Split your site into sections based on keyword research.
Your index should emphasize on the most important and general keyword.
The deeper you enter your site, the more specific your keywords should become.

Programming > C++ Programming > Objects in C++ Programming
> Programming C++ Functions
> BASIC Programming

Narrow versus broad keyword targeting
Every page on your web site must take one of the two basic approaches on keyword targeting.
A page can either concentrate on one specific term or cover a few similar ones.
When optimizing a page about "computer programming" using the broad approach, you can simultaneously target analogous phrases like "programming for computers", "coding computer programs" and so on.
The broad method will cause you to rank lower but in a wider spectrum of searches.
The narrow approach is about striving to dominate in one specific phrase.
When using the latter method, one must be especially careful of keyword overuse.
Using keywords (or phrases)
Repeating the same keyword or printing a large number of them in a list will not get you far.
Search engines are wary of excessive repetition.
Targeting keywords in a pages should be about tweaking it from a technical standpoint.
It must not interfere with your original content.
Messing around with your body text too much degrades the visitors' experience.
Also remember that without having a good keyword hierarchy, "low-level" optimization is pointless.
Here are the primary places on each page where keywords should be emphasized (in order of importance):

First sentence in the <Body> tag
Inbound anchor text
Outbound anchor text
<H2> tag
Filename or URL path
Meta description
<B> <Strong> and <I> tags
